Your First Visit

Starting at a new dialysis center can feel overwhelming. Here is everything you need — a simple checklist and what to expect on your first day.

Admission requirements

Please bring the following documents on your first session:

  1. Physician's dialysis order
  2. Treatment flowsheet (latest 3)
  3. History and physical and/or Clinical abstract
  4. Progress notes
  5. Chest X-Ray (most recent)
  6. EKG (most recent)
  7. Laboratory results (most recent — not more than 30 days)
  8. Hepatitis profile / status
  9. Face sheet / personal info

What to expect

  1. Call or message us

    Get in touch so we can coordinate your schedule and go over the requirements with you. If anything on the list is missing, we can often help you sort it out before you come in.

  2. Bring your requirements

    On your first session, bring the documents from the checklist above. Our staff will take care of the paperwork, including PhilHealth processing.

  3. Arrive a little early

    Please come 15–30 minutes before your scheduled session so our team can do a short assessment and get you settled comfortably.

  4. Relax during your session

    Each session runs about 4 hours. You can rest, read, or watch TV — our nurses will check on you regularly, and a physician is on duty if you need anything.
